Financial Policies
Please bring your insurance card with you to each visit. Copays are due and payable at the time of your visit. Insurance contracts require that we collect your copay at each visit for each patient seen. If we are a participating provider with your insurance company, we will submit all insurance claims for you. We ask that you pay all coinsurance and deductible amounts at the time of your visit. If your insurance company does not pay for the visit, balances are due in thirty days, unless previous arrangements have been made. We send monthly statements on all outstanding balances. If you have any billing questions or insurance problems, please contact our office manager by phone or through our website.

We update our database each year and will ask you to complete a new patient information form for each child. You can download this form from our website prior to your visit. Be sure to inform us at all visits if you have any changes in insurance, address or telephone number.

Referrals and Insurance Authorization
Your insurance plan may require you to have a referral from us before seeing a specialist, having certain tests or visiting immediate care or the emergency room. We have a referral coordinator who makes these arrangements. Please contact her by calling our main number or through our website if you require a referral. Be sure to allow three business days before your appointment so we can submit all necessary information to your insurance company. Insurance contracts require referrals to be submitted prior to the visit. Thanks for your cooperation!
